Your employer (or the employer of your spouse, partner or family member) allows you to use the Health Management Program described on this Website. Your participation in the Health Management Program is completely voluntary, but if you do participate, we will need to collect your personal information to allow you to use the benefits of the Health Management Service. We collect your personal information from the online questionnaire or via any follow-up communications we may have with you, which may include e-mail, telephone, or postal mail.
